Services in Weaverville and Hayfork Clinics:
Immunizations, pregnancy testing, WIC exams, TB screening, HIV testing and
counseling, general health questions.
Child Health & Disability Prevention
Referral system for complete health and dental
check-ups for income eligible children through 19 years of age. (If Medi-Cal
eligible, through 21 years of age.)
California Children's Services
Case Management for children with significant medical
problems that meet State criteria.
HIV Case Management, Education & Prevention
Provides case management for individuals with
HIV/AIDS. Coordinates HIV education and prevention in collaboration with
various community based organizations.
Maternal Child Adolescent Health
Provides referrals to a variety of services that
protect and improve the health of pregnant women, infants, children and
adolescents.
Peri-natal Outreach Education
Provides women in Trinity County with assistance in
accessing prenatal care; education to assure healthy pregnancies and babies; and
referrals to county resources.
Women, Infants, & Children (W.I.C.)
Provides nutrition education and vouchers that are
redeemable at local grocery stores for nutritious foods. Services offered
to pregnant and breast-feeding women, infants and children under age 5 who meet
income qualifications. Sites in Hayfork and Weaverville.
