Get Access to Health Coverage and information in Trinity County

Trinity County Health and Human Services’ Health Outreach, Prevention, and Education (HOPE) program promotes health and wellness of individuals in order to increase access to health care and decrease preventable hospitalizations.


Medi-Cal 

Medi-Cal provides quality health care coverage to eligible residents of California with income and resources below established limits.

Medi-Cal Eligibility:

If your family has income at or below 138% of the Federal Poverty Level (FPL) (266% of FPL if you're a child), you may be eligible for Income-Based Medi-Cal.

You can also get Medi-Cal if you are: 

  • 65 or older 
  • Blind 
  • Disabled 
  • Under 21 
  • Pregnant 
  • In a skilled nursing or intermediate care home 
  • A parent or caretaker relative or a child under 21 if: 
    • The child's parent is deceased or doesn't live with the child, or
    • The child's parent is incapacitated, or     
    • The child's parent is under employed or unemployed 
  • Have been screened for breast and/or cervical cancer

County Medical Services Program

Eligibility:

Adults between 21 and 64 years old
Legal residents & undocumented persons 
    Applicants can qualify with 
    Incomes up to 300% FPL 
    Keeping more personal assets (up to $20,000 for a single person) 
    Reduced monthly cost-sharing
Persons cannot be eligible for Medi-Cal, Covered California, or Medicare
Coverage in 35 participating counties

CMSP Benefits:

New, added no-cost health care benefits

Up to three (3) office visits for primary care, specialty care or physical therapy

Preventative health screenings & lab tests

Prescription drugs with a $5 co-pay (up to $1500 in benefits per enrollment term)


Covered California

Covered California is a marketplace that allows individuals and families to get free or low- cost health insurance through Medi-Cal, or to get help paying for private health insurance.

Covered California can help you choose a private insurance plan that meets your health needs and budget.

Any Californian can get health insurance through Covered California if he or she is a state resident and cannot get affordable health insurance through a job.

Applicants may qualify for a free or low-cost health plan, or for financial help that can lower the cost of premiums and co-pays. The amount of financial help is based on household size and family income.
